VOLUNTEER AGREEMENT, CONFIDENTIALITY AGREEMENT, MEDIA RELEASE, AND

RELEASE & WAIVER OF LIABILITY

Volunteer Agreement

In return for being allowed to participate in Glowing Hearts Charity activities as a volunteer (e.g.: board member, committee member, or for other volunteer activities) the undersigned volunteer (or parent/legal guardian if the volunteer is under age 18) agrees to the following:

I acknowledge that my activities are performed on a volunteer basis, that there is no employment relationship with Glowing Hearts Charity, and the organization does not carry any insurance of any kind for my benefit.

If required, I agree to provide a criminal reference check and/or vulnerable sector check.

I understand that to participate in any activity that involves lifting (i.e.: backpack packing and delivery, event set-up, and organizing inventory), I am able to lift up to 30 lbs and will not lift an amount more than I am comfortable with.

As with any task, there may be a degree of risk. After careful consideration of potential risks, I will take responsibility to ensure my own safety, and I acknowledge that I have a right to refuse to engage in any work that I feel is not safe.

I suffer no impairment that would or should prevent my participation in volunteer activities, and should my health change, I will notify my supervisor.

Glowing Hearts Charity has various policies in place promoting a safe environment for its members. I agree to abide by all Charity policies, guidelines, and procedures, and ask for clarification if unsure.

I understand that my obligations and commitments set forth in this agreement are important to ensure the integrity of Glowing Hearts Charity and as a result, a breach of this agreement may cause my immediate termination.

I acknowledge, while as a volunteer, that I have been strongly encouraged to carry adequate medical and/or health insurance. I am responsible for my own health, dental, and medical coverage.

If I will be operating an automobile in the course of my activities, I acknowledge it is my responsibility to ensure adequate automobile insurance coverage.

I have been provided with, had the opportunity to review, and will comply with all policies and procedures found in the volunteer manual.

Confidentiality Agreement

In the course of performing my duties, I may have access to confidential information, including but not limited to:

Written, verbal, and electronic communications concerning the business and operations of Glowing Hearts Charity;
Member, donor, and agency information (name, contact information, corporate, and donation amounts); and
Client information (name, contact information, receipt amounts).

I agree to observe and comply with Glowing Hearts Charity’s privacy of information policy, and their rules and regulations respecting confidentiality.

This duty to protect confidential information extends beyond the termination of my association with the Charity.

Media Release

I hereby GIVE consent to the Glowing Hearts Charity and its agents or representatives to take photographs, videos or audio recordings of me to promote, publicize, or explain Glowing Hearts Charity and its activities and functions, for future presentations, funding reports, applications, and on social media without any further approval from me or any payment to me.
